A: Medical records can be released or obtained by written consent only. Therefore, patients wishing to transfer their medical records to or from Christ Clinic must sign a Release of Records. This form can be obtained from Christ Clinic or at their previous/future provider’s office. Christ Clinic contracts with a local company to oversee the secure transfer of records from Christ Clinic to other offices or to the patient. Thus, a 2-3 three week lapse may occur between the time the Release of Records was signed, and when the records are received by the desired party.

Q: I’m not a patient, but need to be seen urgently – can I come to Christ Clinic?
A: You can call to see if there are any new patient appointments available, however there is generally a 4-6 week wait for a new patient appointment.

Q: Can I schedule an appointment for someone else?
A: In order to help prevent no-showed appointments, and to protect patient information, Christ Clinic does require that adult patients call to schedule their own appointments. If this is impossible due to a language barrier, please call and speak with our Receptionist.

Q: What happens if I miss my appointment?
A:
Christ Clinic considers a no-showed appointment to be one missed with no notice or less than a 24-hour notice. If a patient no-shows their first appointment at Christ Clinic, they cannot be rescheduled. If an established patient no-shows their appointment, they must be scheduled for 11:00 am appointments until an appointment is successfully completed.

Q: What happens if I’m late to my appointment?
A: We understand that sometimes, unforeseen events prevent patients from arriving to their appointment on time. For this reason, Christ Clinic offers a ten-minute grace period to late arrivals. Once the 10-minute window has closed, the appointment must be rescheduled and will be considered a no-show.
